This is a very rare fossil. It is an Unidentified Larva (Pupa?) from the Eocene of Utak. Fossils like these are extremely rare in the fossil record. This specimen has nice detail intact - the segmentation is easily seen. The fossil displays beautifully on thelarge, squared plate of shale. A distinct insect preserved on shale is very uncommon, but a larva or pupa from the Green River Formation of Utah is quite rare.
